Use this form to apply for support from
The Phil Zhu Foundation.
Before starting your application, please make sure you have a recent photograph of the applicant ready to upload. You may also need to provide supporting evidence depending on your circumstances.
Applicants must be under 18 years of age and based in the Merseyside region.
The Foundation has limited funding available and may not be able to meet all applications. Applications may not be considered if required evidence is missing.
You may need to provide:
• A recent photograph of the applicant
• Evidence of benefits, such as DWP communication or Universal Credit statement
• Evidence of employment income, such as payslips, bank statement or employer letter
• Proof of dependants, such as birth certificate, passport or child benefit statement
• Proof of full-time education for dependants aged 16+
• Proof of rent or mortgage, such as rent agreement, bank statement, lender letter or mortgage agreement
